PMH-BC is the board certification for registered nurses who work in psychiatric-mental health. The American Nurses Credentialing Center (ANCC) awards it to RNs who have practiced the equivalent of 2 years full time, logged 2,000 hours of psychiatric nursing in the last 3 years, completed 30 hours of psych continuing education, and passed a 150-question exam. It costs $220 to $395, lasts 5 years, and isn't required to work as a psych nurse. It is the main way to show you know the specialty.
The letters stand for Psychiatric-Mental Health Nurse, Board Certified. Don't confuse it with PMHNP-BC, the nurse practitioner credential. Both come from ANCC and share most of a name, but they belong to different roles.
What Is PMH-BC Certification?
PMH-BC is ANCC's Psychiatric-Mental Health Nursing certification. ANCC describes the exam as a check of the entry-level clinical knowledge and skills of RNs in the specialty after initial licensure. In practice it's taken by nurses on inpatient psych units, in crisis and emergency psychiatry, in addiction treatment, and in community mental health.
It's a specialty credential, not a license. Your RN license is still what lets you practice. PMH-BC doesn't widen your scope: you can't diagnose or prescribe with it, and your duties are the same the day after you pass. What it changes is how employers and graduate programs read your resume. The Accreditation Board for Specialty Nursing Certification accredits it.
It's also a small club. ANCC counted 10,900 PMH-BC holders at the end of 2025, against 58,578 certified PMHNPs.
PMH-BC Eligibility Requirements
ANCC lists four requirements. You have to meet all of them when you apply.
- A current, active RN license in a U.S. state or territory, or the legal equivalent in another country.
- The equivalent of 2 years of full-time RN practice. Any specialty counts toward this part.
- At least 2,000 hours of clinical practice in psychiatric-mental health nursing within the last 3 years.
- 30 hours of continuing education in psychiatric-mental health nursing within the last 3 years.
Two things aren't on the list. ANCC doesn't ask for a bachelor's degree, so an RN with an associate degree can sit the exam. And there's no required course. Any psych CE that fits the 3-year window counts.
The 2,000-hour rule is the one that sets your timeline. At three 12-hour shifts a week, it takes about 13 months of full-time work on a psych unit. If you started in med-surg, your first two years count toward the RN practice requirement, but only your psych hours count toward this one. Have a copy of your RN license showing its number and expiration date ready to upload.
The PMH-BC Exam: Format, Content and Passing Score
The exam is computer based, and you take it at a Prometric test center. You can apply any time of year. Once you're approved, you get a 120-day window to test.
You get 3 hours for 150 multiple-choice questions. 25 of them are unscored pretest questions that ANCC is trying out, and you can't tell which ones they are, so answer everything. Your result rests on the other 125.
What the 125 scored questions cover, from ANCC's test content outline.
| Domain | Questions | Share | What it covers |
|---|---|---|---|
| Assessment and diagnosis | 28 | 22% | Developmental stages, physical causes of psychiatric symptoms, psychiatric disorders, coping and defense mechanisms, the mental status exam and safety risk assessment |
| Planning | 26 | 21% | Client-centered care, teaching and readiness to learn, cultural competence, communication barriers, treatment planning |
| Implementation | 58 | 46% | Treatment modalities, milieu and safety, therapeutic communication, medication management, group facilitation, crisis management |
| Evaluation | 13 | 10% | Legal and ethical issues, informed consent, documentation, process improvement, outcome measurement |
Implementation is almost half the scored exam. That's the daily work of a psych unit: milieu and safety, therapeutic communication, medication management and crisis response. Medication questions sit here as reconciliation, side effects, contraindications and lab values. You're tested as the nurse who gives and monitors the drug, not the one who picks it.
ANCC reports a pass or fail. Scores sit on a scale that tops out at 500, and you need 350 or higher to pass. ANCC converts the number you got right to that scale. Results reach ANCC within 24 to 48 hours. ANCC's own test content outline is worth printing before you start studying, because every question maps to it.
Plan to study. 71% of the 1,366 first-time test takers passed in 2025, which is 11 points below the PMHNP exam.
How Much Does PMH-BC Certification Cost?
The first exam costs $395 if you aren't a member of a nursing association, and less if you are.
ANCC's PMH-BC fees. Each includes a $140 administrative fee that isn't refunded.
| You are | First exam | Renewal |
|---|---|---|
| Non-member | $395 | $350 |
| American Nurses Association member | $295 | $250 |
| American Psychiatric Nurses Association member | $220 | $250 |
| International Society of Psychiatric-Mental Health Nurses member | $340 | $295 |
The American Psychiatric Nurses Association discount is the biggest one. It takes $175 off the non-member price, so check what APNA membership costs you before applying. You can claim a discount for up to 5 business days after ANCC receives your application, not later. Some employers reimburse certification fees or pay for a review course, so ask before you pay.
How to Apply for PMH-BC
- Check your hours. Count your psych hours from the last 3 years and gather certificates for 30 hours of psych CE.
- Read ANCC's certification handbook. It covers scheduling, ID rules and deadlines.
- Apply online at ANCC, and claim your membership discount.
- Schedule with Prometric using your ANCC certification number, inside your 120-day window.
- Sit the exam. Your certification starts on the day you pass.
If You Don't Pass
You can retest 60 calendar days after your last attempt, and you can't test more than 3 times in any 12-month period. You'll file a retest application, pay the retest fee and take the whole exam again. A failing score report includes diagnostic information showing where you fell short, so use it to aim your second round of study.
How to Renew PMH-BC
PMH-BC lasts 5 years. You can file for renewal up to a year before it expires. Under ANCC's renewal requirements you need to:
- keep an active RN license
- complete 75 contact hours of continuing education
- complete one of 8 professional development categories in full, such as academic credits, presentations, precepting, professional service, or 1,000 practice hours in the specialty over the 5 years
- pay the renewal fee
Most working psych nurses renew through the practice-hours category, since 1,000 hours over 5 years is a fraction of a full-time schedule. Retaking the exam also counts as a category. Since January 14, 2026, every renewal activity has to fall inside your current 5-year certification period. Hours from before it don't carry over, so log them in your ANCC account as you go.
Does PMH-BC Increase Your Salary?
Sometimes, depending on the employer. No public survey separates certified from uncertified psych nurses, so any precise "PMH-BC raise" you see quoted is a guess. What you can check is your own employer's clinical ladder. Many hospitals tie a ladder step, a certification differential or an annual bonus to specialty certification, and the amount is written into the policy or union contract.
For the baseline, the median registered nurse earns $97,550 a year, according to BLS OEWS, May 2025. Psych nurses are paid on the same RN scale. Our psychiatric nurse salary guide breaks that down by state and setting and shows where certification fits among the other ways to raise your pay.
PMH-BC vs. PMHNP-BC
These are different jobs, not two levels of one certification.
PMH-BC and PMHNP-BC side by side. Pass rates are ANCC’s 2025 figures for first-time test takers.
| PMH-BC (RN) | PMHNP-BC (NP) | |
|---|---|---|
| Who holds it | Registered nurses working in psychiatric-mental health | Psychiatric-mental health nurse practitioners |
| Education | RN license; ANCC lists no degree requirement | A PMHNP master’s, DNP or post-graduate certificate |
| Clinical hours | 2,000 hours of psych RN practice in 3 years | 500 or more faculty-supervised hours in the program |
| Diagnose and prescribe | No | Yes, under state APRN law |
| Needed to practice | No, it is voluntary | Yes, in most states, for APRN licensure |
| 2025 first-time pass rate | 71% | 82% |
Plenty of PMHNPs held PMH-BC first. It's not a prerequisite for a PMHNP program, but the psych hours behind it are exactly the experience admissions committees want to see. Our PMHNP certification guide covers the nurse practitioner exam, and the PMHNP board exam guide covers how to prepare for it.
Is PMH-BC Certification Worth It?
If you plan to stay in psych nursing, usually yes. For $220 to $395, often paid by your employer, you get a credential that fewer than 11,000 nurses hold. It can unlock a ladder step and makes you more competitive for charge, educator and outpatient roles. Studying for it also fills the theory gaps that a few years on one unit leaves.
It's less useful if you're about to start a PMHNP program. The NP board exam replaces it, and you'd be paying for a credential you'll stop using once you're licensed as an APRN. Still, if you're a year or two from applying, it's a strong line on an application and good practice for a high-stakes ANCC exam.
If PMHNP is where you're headed, see PMHNP vs. psychiatric nurse for how the two roles differ day to day. From there, the routes are BSN to PMHNP and, for nurses with an associate degree, RN to PMHNP without a BSN. The full RN path, from nursing school to your first psych job, is in our guide to becoming a psychiatric nurse.
